Today’s Vlog: Pelvic Floor Dysfunction Treatment with Prolotherapy Injections
How does Prolotherapy help pelvic floor dysfunction? This is kind of one of my interests because whether it’s a pubic problem, a pelvic floor problem, a lower back problem, it is very disabling. I would say basically almost every day at Caring Medical there is someone coming in with a pelvic floor problem. I’d like to just show this. This is basically the pelvic floor. (holds up pelvic model) The pelvic floor goes from anteriorly we have the pubic symphysis here and then you have the pubic rami. This is the ischial tuberosity. The pelvic floor is all the musculature here.
The normal symptoms that folks have is just terrible pelvic floor problems and then that can lead to erection problems, problems with intercourse, urination problems, problems holding urination, bowel problems, and the pain is just awful. From a relationship standpoint, it interferes with relationships.
I had a discussion the other day with a client who came in from Houston. She was a wonderful nice young gal, newly married, and it was causing a lot of problem in her marriage because it was just very painful. She was asking about food allergies and physiotherapy and all these things and I said to her “if you and I were going to write a comprehensive paper on pelvic floor, and we are going to research it, what would be one injury that could cause you pubic pain,pelvic floor pain, urinary problems, screw up the nervous system in the pelvic floor?” I think that we would come to the conclusion that it is pelvic instability. If you have pelvic instability it means that you have a injury to the ligament support. The ligament support that causes these bones to stay in alignment.
Let’s just say you had a pubic symphysis problem. You don’t even know you have it. All you know is that you slipped on the sidewalk. You slipped on the sidewalk, you didn’t think much of it, and now all of a sudden you have pubic symphysis. Well guess what’s going to happen when you try to pee? Those muscles try to contract. You have to realize you only have good control if they contract against a stable base. So, while your pelvic floor muscles are trying to contract and your pubis is unstable you are going to go into muscle spasms. When you get muscle spasms, the pain is excruciating, and obviously anything that puts pressure on theligaments like intercourse, lunges, or running, any sort of activity that is going to put pressure on the ligaments and that is going to make the symptoms worse.
Patients with pelvic floor dysfunction will tell you they can’t do crunches, they can’t do a lot of different physical activities, because the pain is excruciating. We have very good success treating pelvic floor dysfunction, pubic problems, ischial tuberosity problems, with Prolotherapy. Specifically, I would poke on the various areas. So, if I was poking on somebody’s pubis and it was tender I would treat that, the pubic ramus if that was tender. This is the anterior part of the ischial tuberosity. We might treat the anterior and posterior parts of the ischial tuberosity. Sometimes we might have to treat the lower back. You might think “I have a pelvic floor problem, I don’t have ligament instability in my back.” Well, the ligament instability in your back could be causing the pelvic floor problem. If you have clicking in your back, if you raise your legs and all of a sudden the back clicks, if you have had a lot of chiropractic care. I was talking to the gal from Houston and I said “First of all, most of the pelvic floor problems are in females. That’s what I see anyway. Two, is almost all of them have hypermobility.”
If you have hypermobility that means you are a loose jointed gal. You have more flexibility than the average person and you have pelvic floor pain, most likely you have ligament injury in your pelvis. You have pelvic instability and you have to stabilize it. This particular person had had a whole bunch of care before she saw us nothing worked, and she was disabled. She was here for a second visit and she was already 25% better. You could tell. Her husband was here, she was here, and she was just so much happier. There is a good chance that she will completely get cured with Prolotherapy.
